Abstract
pilepsy surgery is a highly effective therapy with the aim of long-term seizure control,i.e., to cure the epilepsy. Three different
studies with class 1 evidence have shown this. In patients with drug-resistant epilepsy (DRE) lasting for nearly 20 years, shorter
disease duration of about 5 years, or in children it has been shown independently that epilepsy surgery is highly superior to
best medical treatment. Using multimodal techniques (e.g., long-term video EEG, highresolution
cerebral MRI according to epilepsy
protocol, FDG-PET, neuropsychological testing, fMRI, ictal SPECT with SISCOM, MRI
postprocessing, electric source imaging [ESI],
magnetoencephalography [MEG], invasive recording with stereo-EEG electrodes or subdural electrodes and WADA test), it is possible nowadays to offer epilepsy surgery
to patients who were previously not regarded as promising candidates for epilepsy surgery.
