Description

Management of chronic diseases requires long-term coordination of care plans across different health providers. Current IT support is typically restricted to data management, since currently available workflow products can only cope poorly with specific challenging requirements of coordinated health care, such as process individualisation and inter-process recognition of similar activities already performed or scheduled in another process. The project will develop concepts and techniques to support inter-health-provider care plans through workflow technology and use healthcare coordination projects of the South Australian Department of Human Services as case studies in a proof-of-concept prototype.
